This happened in two matches one after the other before I decided to play as a different killer.

Steps to Reproduce
- Turn to killer instinct ping to be able to teleport to box
- Teleport to box
- If you don't fall through the map or fall through and ping back up you are fine
- If you fall through map see if you are then stuck in floor, unable to move.
Explanation
When teleporting to lament's configuration box being solved by survivor Pinhead can either fall through the map and bounce up normally or can teleport and become stuck in the floor. He then cannot teleport or move for the rest of the match but can still turn, hit things when in proximity and throw poison in the event and summon chains, however, he is stuck in the floor or falls through.
