Steps to Reproduce

1. Hosted a D3 run
2. Got one person before launch
3. One from the toilet
4. The phantom 4th member showed up later

Explanation

4th member joined the mission late on the 1st level. They never showed up in the lower-left display or in the post-loss screen lineup.
