Steps to Reproduce

1. Get to boss
2. Activate all rift stones
3. Finish first phase and throw in rift stone
4. Finish second phase and throw in rift stone

Explanation

I was fighting the Gotoorak Gatekeeper. We had thrown one rift stone in, and then when I threw the second stone it took out his 3rd HP as well.
