Steps to Reproduce

1. Just appeared seemingly out of nowhere, no clue how it happened.

Explanation

OMEGA appears to be stuck in the level announcement line, the 'subtitles' wouldn't disappear and no other line could be played. I did not fix itself when entering a new level. Also all other audio from the game appear to be lower than usual.
