Steps to Reproduce

  1. Misson went critical as we started the final boss fight
  2. worm spawned in front of me
  3. I strafed as he lunged toward me
  4. his head went underground
  5. he grabbed me through the floor

Explanation

The giant worms that spawn during red/critical can put their head through the floor and still grab you while they're underground and dont retract while being damaged.
