Steps to Reproduce

1. Play as Pyramid Head / The Executioner.
2. Fire a POTD attack without the addon
3. Fire a POTD attack with the addon
4. Compare the speed of both attacks

Explanation

Pyramid Head's Forgotten Videotape Add On has no effect. It is supposed to increase the travel speed of your ranged attack by 10%.

Testing the addon multiple times with similar setups shows no speed increase, even having hits register at the same time down to a frame. The add-on is completely useless and has not functioned since the Add-On pass happened.
